Intermountain Centers is a private, not-for-profit service organization, which provides residential, home-based, and transitional support services to a variety of clinical populations. Intermountain programs are designed to develop interpersonal and social skills, self-sufficiency and self-management skills, academic and vocational skills, community awareness and responsibility and constructive future planning.

Intermountain is currently recruiting for a Therapeutic Foster Care (HCTC) Program Coordinator, for which the job description is posted below:
JOB RESPONSIBILITIES
Attendance -
-Works a flexible schedule to meet program needs; follows agency attendance policies; participates in on-call rotation

Therapeutic Treatment Model -
-Ensures the integrity of the behavioral model, as outlined in the most recent ICHD program description, is maintained by HCTC providers
-In coordination with HCTC providers, coordinates implementation of individual behavior plans, safety crisis plans and Individual Treatment Plans (ITP‚s) for clients in placement

Behavioral Intervention -
-Works with HCTC providers to implement proactive treatment interventions, including behavioral contingencies; tracks outcome measures.

Provider Support-
-Provides leadership and support to HCTC providers
-Conducts, at minimum, daily phone contacts, weekly visits to the homes, and crisis services for HCTC providers

Provider Oversight-
-Ensures that provider homes are maintained in accordance with licensing and contract standards.
-Ensures client files are maintained in accordance with agency and contractual standards
-Monitors communication between HCTC providers and families, officials and professional workers

Client Support-
-In collaboration with HCTC providers, coordinates the planning and implementation of client‚s academic and/or vocational program
-Ensures continuity of client‚s treatment program by monitoring completion of functional assessment, development of individual goals, completion of progress reports and other required documentation
-Develops and maintains professional relationships with HCTC providers, professional workers, clients‚ family members, and any other stakeholders necessary
-Confers monthly with individual caseworkers and clients‚ families

Recruiting/Home Studies-
-Assists in recruiting of HCTC providers
-Assists in assessing and evaluating potential HCTC homes for home studies, pre-placement visits, licensing standards and suitable placement matches between clients and care providers

Training-
-Assists in initial and ongoing training of HCTC providers

Staff Meetings-
-Participates in weekly staff meetings

Clinical Supervision-
-Participates in weekly supervision with Director of Foster Care

Crisis Response-
-Provides 24/7 crisis and support service to assigned HCTC homes(may include transporting clients.)
-Provides emergency back-up to all HCTC homes as determined by on call schedule (may include transporting clients.)

Fiscal Responsibility-
-Follows agency purchasing and accounting procedures.

Skills / Requirements

EDUCATION, EXPERIENCE AND LEGAL REQUIREMENTS

Education/Experience-
-Master‚s degree in a field related to Behavioral Health preferred
-Bachelor‚s degree in a field related to Behavioral Health, and three years‚ full-time Behavioral health work experience, or
-Any Bachelor‚s degree and five years‚ full-time behavioral health work experience required.
-Prior related experience preferred

Driver Requirements-
-Arizona Driver‚s License and 39-month driving record that meets insurance requirements
-Own reliable transportation for transport of self or others for business purposes (ICHD provides mileage reimbursement)
-Proof of liability coverage of $100,000/$300,000 personal damage liability, and $50,000 property damage liability.

Legal/Licensing Requirements-
-21 years of age
-CPR, First Aid , and Crisis Intervention (CPI) certification required (ICHD provides training)
-DPS Level 1 fingerprint clearance required
-Physical examination report including a current negative TB test result required (ICHD provides)

Important Notes

Bilingual candidates encouraged. Bi-lingual differential of $1,500 paid in addition to base salary for competency-tested staff.
