Jobing Description

1. Provides outpatient education and counseling to individual patients and families in accordance with established treatment/educational plan
2. Develops, implements and evaluates individualized patient educational plan based on assessed need(s) to ensure needs are met
3. Develops educational materials in response to physician requests based on patient treatment needs
4. Performs case management of self-care diabetes program participants to ensure completion of annual physical assessments including, but not limited to: eye, dental and podiatric
5. Schedules and organizes group classes including, but not limited to: arranges for speakers, demonstrations, room and dietary set-up to present healthy cooking, weight management and lifestyle alternatives
6. Track, notify and facilitate assigned self-managed diabetes clients through program in accordance with teaching plan and personal goals
7. Assists procure, organize and maintain inventory levels of printed and audiovisual materials to ensure ongoing availability of current information
8. Participates in assessment of community needs for educational services; designs and implements programs to meet needs
9. Assists review/revision of all standards, policy and procedure manuals to ensure ongoing adherence with requirements
10. Reviews program materials and supplies for relevancy and usefulness; recommends changes as needed
11. Works cooperatively and collaboratively with tribal members to address program outcomes and integration of outreach efforts
12. Develops and delivers diabetes educational programs and workshops for various tribal entities to ensure continuous awareness and understanding
13. Serves as diabetes education resource to other health care providers, patients and patient‚s families by providing information in response to inquiries regarding services and program availability
14. Works closely with Social Work, Public Health Nursing, Business Office and Case Management to identify appropriate community resources; follows up with patient and/or family regarding service level(s)
15. Participates in departmental orientation, on the job training and quality assurance programs/initiatives
16. Participates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor

Skills / Requirements

QUALIFICATIONS:
Current licensure as a Registered Nurse or Registered Dietician in the State of Arizona

Bachelor‚s degree in Nursing, Nutrition or closely related field

Must have at least 2 years previous diabetes education experience; Certified Diabetes Educator or CDE
