Position Description:

Candidate will work under direction of an RN. Candidate will provide direct care such as but not limited to bathing, grooming, dressing, feeding, and delivering supplies to our patients in their homes or in a nursing facility according to the plan of care.

Positions in this function require various clinical licensure/certification based on role. May include: RN, LPN/LVN, CNA. Non-RNs work under direct supervision of someone with RN or MD licensure as part of an interdisciplinary team. Function is responsible for overseeing and participating in day to day clinical care of terminal patients and their families. Function is also responsible for clinical care and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating) as it relates to end of life palliative care.

Primary Responsibilities:Moderate work experience within own function.Some work is completed without established procedures.Basic tasks are completed without review by others.Supervision/guidance is required for higher level tasks.Requires current unrestricted CNA licensure in applicable state.Requirements:

HS Diploma or GED
Must be a licensed CNA
1 + year of CNA or Caregiver experience
Must have valid driver's license, reliable transportation and auto insurance
Must be CPR certified
