Summary:
Under the direction of the Director of Utilization Management, responsible for the supervision of the Linkage Liaison Team who, in conjunction with the member‚s adult recovery team, provide immediate assistance, evaluation and wrap-around services to high risk/high need individuals transitioning out of the hospital to outpatient services. Goals include: decreasing readmission rates, avoiding higher levels of care (Level II, etc.), new enrollment orientation to CODAC services, linkage to natural supports, community and outpatient services. In addition, the UM Coordinator is responsible for assisting the agency in providing possible Level I diversion opportunities, managing utilization of supported housing funding, and providing limited evening and weekend hour on-call clinical support and supervision.

Essential Duties and Responsibilities:

  • Monitors staff implementation and compliance with agency policies and procedures.
  • Provides supervision and coaching to up to seven Linkage Liaison Staff.
  • Prioritizes Linkage Liaison referrals, tracks progress, and ensures follow-up and follow-through to optimize clinical outcomes.
  • Directs assigned program activities of Linkage Liaison staff while ensuring compliance consistent with agency and funding source requirements.
  • Functions as daytime administrator on call to divert Level I admissions (when appropriate).
  • Monitors and implements procedures related to supported housing subvention funds under supervision of the UM Director.
  • Attends requested agency and community meetings and communicates pertinent information to appropriate staff. Upon request performs a functionary role in ECU Staffing and the Pharmacy and Therapeutics committee.
  • Prepares and submits timely and accurate reports and other documents related to program services and activities.
  • Represents the agency at meetings and professional or community functions.
  • Reviews and prepares reports for the purpose of monitoring assigned program activities and functions.
  • Coordinates services with other behavioral health agencies, service organizations and other community resources.
  • Performs other related activities in accordance with agency growth and changes.

    Skills / Requirements

    Position Requirements (at hire):
    Education and/or Experience: A combination of one of the following education and work experience are required:

  • Bachelor‚s degree in a non-behavioral health field AND 30 semester hours in behavioral health education, plus 4 years behavioral health experience; OR,
  • Master‚s degree in behavioral health field; OR,
  • Master‚s degree in a non-behavioral health field AND 30 semester hours in behavioral health education, plus 2 years behavioral health experience.
    Certifications:
  • Arizona Behavioral Health Licensure, preferred
    Additional Requirements:
  • Valid Arizona Driver‚s license, proof of current insurance and willingness to use personal vehicle. Driver‚s license is an essential requirement for performing duties for this position.
  • Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
    Other:
  • Extensive knowledge of community resources.
  • Extensive knowledge of Hospital System of Care
  • Knowledge of behavioral health challenges which include knowledge of the impacts of mental illness, domestic violence, and the dynamics of addictive behavior.
  • Familiarity with various ethnic and cultural groups.
    Skills:
  • Ability to communicate effectively both orally and in writing.
  • Ability to interact effectively with other service providers.
  • Intermediate to advanced computer skills using MS Office products, Word, Excel, Access, etc., importing/exporting data to/from applications.
  • Demonstrates clinical competency in accordance with standards for the position.
  • Clinical knowledge of behavioral health disorders, across all populations (persons with serious mental illness and chronic substance abusers).
  • Knowledge of psychosocial rehabilitation and recovery models.
  • Experience in the areas of supervision and management.
  • Knowledge of managed care systems and AHCCCS/Title XIX medical necessity criteria.
  • Ability to interact and provide effective leadership in collaboration with other behavioral health professionals.
